Apply for a Lifetime Achievement Award for a very successful show cat.

1. Go to http://TFMS.tica.org
2. Click on My Cats
3. Open the cat who has earned this award
4. Click “Request Service”
5. Select “Request Confirmation for Outstanding Dam/Sire or Lifetime Achievement Award”
6. Select “Confirmation (Lifetime Achievement Award – 8×10 Laminated Certificate)”
7. Follow the instructions (download, complete, and upload the form)
8. Go to the shopping cart and pay the $20 fee
